Membership Terms

Chosen Health memberships are month-to-month or annual agreements providing access to provider visits, lab cadence, personalized supplements, and member pricing as described in your enrollment.

Cancellations are accepted at any time. Annual prepaid memberships may be eligible for prorated refunds at the discretion of Chosen Health.

Membership benefits are not insurance and do not replace insurance coverage. Some services may require separate fees. Specific therapies (e.g., GLP-1, HRT, PRP) are provided only when medically appropriate and at the discretion of your licensed provider.
